Ref getting injured question

Watching the Rugby League on BBC 2 and they were saying that the ref had done something to his calf during the warm up and the the TMO was going to be ref and the original ref was going to be the TMO

I was wondering if this has ever happened in Rugby Union, where the match ref has got injured pre game or during the game? I think I remember some of the 3 officials having to be changed a number of years ago at Ravenhill, but I could be wrong.

Toulouse v Perpignan

Chris White was injured in the first quarter of the match and was replaced by Spreadbury.

I have a cloudy memory of both Wayne Barnes and Simon McDowell being used as replacements also.

I also think there was one at Ravenhill but I cannot recall any details.

Ulster v Glasgow at Ravenhill about 4 or 5 years ago.

I can't remember who the ref was but I do remember it was the first game I took my eldest to. He was used to going to Irish League football matches and couldn't understand why the crowd was applauding as the ref was carried off, he was more used to "let him die, let him die, let him die" if an opposing player or the ref had gone down injured.

Last week against the O`s Touch judge cried off injured. >TH local guy stood in
