At Antrim hospital, probably a queue inside trying to get out!
I know people who in the past ordered a taxi to take them from a ward bed in Antrim down to A and E at the Royal!

There lies the problem with the NIHS. Too many shyte provincial hospitals, too many health boards and too many conflicting parochial interests on the Executive to do anything about it

A neat summary Merv of the 2000 report by Maurice Hayes, altho I think he used slightly more diplomatic language! There is no bigger example of the incompetence and failure of devolved power sharing government in Northern Ireland. Good coverage on the slugger otoole site on the topic

Sure it doesn't matter if the hospitals are under funded as long as we have street signs in Irish and an education system in triplicate.

Sorry, no it's not. And it's not money either. By definition, if you have too many hospitals , you will never have enough staff to staff them. Every review of NI NHS since 2000 has said the same thing. We need fewer, centralised,specialised hospitals with outliers to deal with the rest.
As Mervn says above it needs very tough non parochial decisions, a feat which a string of NI self centred parties had shied away from . Why in a 10 department structure, was Health 8th or 9th to get picked!
Cos it was "too hard"
A bunch of incompetent nobodies, and the weaknesses of our non decision making power sharing "government" continue to shine through in the covid 19 response in Ni.

That's what we have because this already happened. In 2006/07 trusts amalgamated to form the 5 we have now. We have seen a significant downgrading of services since 2000 with closures of many essential services such as numerous A&E depts. There has been a major shift towards greater community and outpatient services to decrease hospital admissions (Transforming Your Care 2011).

What services do you downgrade and which hospitals do you now close?

How does this achieve safe staffing levels without reducing hospital bed numbers?

How do you reduce hospital bed numbers when current occupancy is at 105%?

we need to plan strategically for the next 5/10/15/20 years....not next week.

We need to reduce main specialist hospitals from 10/11 down to to 4 or 5

we need to improve infrastructure so that these main hospitals are within 30/40 mins across Northern Ireland etc etc

we need to educate NI population that not everybody can have a hospital on their street corner ( or school for that matter)


But simply wishing that we can have more money and more staff is pie in the sky. Never going to happen.

The funding is there many posts are simply lying vacant and it is possible to train more healthcare professionals. Taking nursing for example, the two universities in NI have both increased their intake for this academic year. This has been raised many times with the executive but we had 3 years of no local government. It is going to take 3 years for these extra places to make any effect. In previous years demand to undertake nursing degrees has well exceeded available places. People have headed across the water and many not returned. There has been significant ongoing overseas recruitment but this has still fallen short.

A bit like Ulster...

Absolutely on that score. Retaining talent and trained people in Northern Ireland across all professions is a much deeper problem than the Health Service, and undoubtedly has a much deeper reason which we will not be able to explore on this thread. There are more "victims" of the Troubles than those killed and injured, and the performance of our local lot could not be persuading many talented people to stay.

That already happens. We may not always agree with what that planning does, but it does happen.

For some "specialisms" you could even go down as far as 1 rather than 4-5, particularly when numbers of patients are not high. Supported by 3-5 sub-regional units if necessary.

We don't need "these main hospitals" within 30/40 minutes across Norn iron. We need these hospitals to have a minimum number of patients for each specialism so that the hospitals (by that I mean staff) build/maintain experience and expertise.

We do need to educate NI population that not everybody can have a hospital on their street corner. I have every sympathy with those who say they have to travel an hour or more to visit loved ones, but I would ask them this, "would you rather have easier visiting but inferior treatment for their loved one because their local hospital is staffed with general surgeons, and other staff who are not specialised nor experienced in the treatments sometimes required?"

If people can understand that, I think most would prefer better treatment for patients even if it causes more inconvenience for visitors, and even patients themselves.
