Pretty sure it was Caldwell for stamping on an Embra player.
Fez and Afoa got citings ( kind of retro reds) but not red carded AFAIK.
The other ones I recall more recently were against us. Grewcock for Bath and a Dragon for kneeing somebody in the head at Ravers.

You're thinking of the classic 15-15 draw in Galway when the great Emerick deserved red but got yellow for GBH on a Weshtie.
